    • Is there partial coverage (Coinsurance)?
    • Is there a Deductible? How much is it? Have I met my deductible?
  6. Do I need precertification or preauthorization?
  7. Ask for a reference number to confirm that this information was documented.
Note: For PPO plans with Anthem Blue Cross, Blue Shield, and Blue Cross Blue Shield; if nutrition services are a covered benefit; then request an authorized “Out of Network Referral”.
